The thing with the names

Indoril Nerevar Indoril.. what?

Actually it's rather simple. Dunmer naming goes like House Name - First Name - Family Name. In cases of nobility, the House Name is usually left out because it can be figured from the family name. In case of a Noble becoming member of a different House, the Family name will be dropped. 

Examples:
When Navradas Taryl became a Telvanni, his name changed to Telvanni Navradas Taryl.
When Ravani Indoril became a Telvanni, her name became Telvanni Ravani.
The full name of Athelyn Demnevanni is Telvanni Athelyn Demnevanni, but since Demnevanni is to Telvanni what Lancaster is to Tudor, nobody mentions it.

Why do they sometimes put it upside down?

When a noble Dunmer addresses another noble Dunmer with their naming order reversed, it's a signal to indicate that they are now talking on a personal level (as opposed to between nobles). It can be either an insult, or a display of trust (or a deliberate "dropping" to a lower "conversation level", for sharing gossip or some such.

Example: I might tell Indoril Nerevar what Nerevar Indoril must never know.
